First let me go over what you need:

1. Computer and Printer or Paper and pen
2. Laminator (go to local bookshop)
3. Ribbon or chain
4. Needle

Its really simple:
1. Choose your design of the tag (bone)
2. Draw it or find a picture of it
3. Write inside of it your dog's name, your name, and phone numbers
If you are using a computer print it out
4. Laminate
5. Create a hole somewhere with a needle
6. Chain or tie the ribbon to your dog's harness/collar

Editor's note: If you do not have access to laminating equipment, you can use clear packing tape to enclose the tag.
